Welcome to the Continuity of Care School

From Semester 2, 2018 first year students will manage their continuity of care relationships data through Sonia. 2019 Third year students will continue to manage their data through the continuity@scu.edu.au email and the program, Conrad.

Students must upload the Consent form to 'Documents' and submit a Midwifery Continuity Partnership Information form via the 'Forms' tab.

Refer to your emails for procedures and screenshots to assist. Procedures will be also be available here on the Sonia homepage.
